Background of Clovis
Clovis is the retail outlet for home monitoring equipment designed and manufactured by SunVision Medical, with manufacturing locations owned and managed by SunVision in China, the Philippines, and the USA. Clovis has the national distribution contract for the complete line of equipment offered by SunVision, which includes the pill dispenser, glucose meter, weight scale, blood pressure monitor, pulse oximeter, thermometer, EKG, and CGM.
The cellular watch features a built-in gateway that enables it to collect vitals from both the watch and standalone equipment, which automatically transfer test results to the watch gateway and upload the data to the health cloud. In 1996, we introduced our first home monitoring device that used telephone landlines
to transfer data collected by our glucose meter. As technology advanced, we were able to offer our Wi-Fi connection device, and now we are offering the cellular device. A few years ago, we conducted a pilot with the State of Georgia to supply our cellular transmission device to patients in FQHC facilities, which helped educate us on what was missing. We believe the solution lies in having the FQ facility function as a support center and be compensated for this service, rather than trying to generate profitable income from offering RPM service.
Equipment Supplied
Eliminating a piece of the supply chain and offering FQ facilities as the supplier enables Clovis to supply more equipment directly to the patient’s home. Offering business this way allows Clovis to supply a complete line of equipment to patients who require it, thereby creating a full disclosure of the patient health. The equipment that can be provided to every patient is the Smart Health router, which connects the doctor to the patient and allows for the collection of vitals through our home monitoring equipment. Business Proposal Pilot
The proposed business model offered to your FQHC market is similar to what is being created through Clovis, and the pharmacy marketplace located in populated areas. Whereas the pharmacy offers home monitoring equipment to be supplied to local doctors patients as needed, in this model, the pharmacy becomes part of the supply chain. The pharmacy also benefits from the after-sales service of the equipment, which includes offering service and customer support as a representative of Clovis/SunVision Wireless. SunVision Wireless has partnered with AT&T as the cellular provider for Clovis home monitoring equipment, operating under the SunVision Wireless brand. The FQHC suggested business model is to offer the facility a contract to support the local doctor patients needs. By providing education on the use of products and educating the patient on their personal responsibilities, if any.
Business Model Created For FQHC
Clovis & SunVision-Wireless would like to offer a pilot with the focus on three states: Mississippi, Georgia, and Florida. Clovis has contractual arrangements that are expected to create a substantial amount of business. We want to send these patients to a local FQ facility to pick up their home monitoring equipment and be shown how to test correctly. The patients will be offered RPM services and the needed equipment, which includes the vital watch, TV Router, and the required FDA-approved equipment to monitor their chronic condition. Here is how SunVision Wireless will reimburse the FQHC facilities for their involvement
as the customer support center.

 $10 is paid monthly to each facility per active patient who is using Clovis equipment.
 The expected volume of patients is 5,000 people per location, which will be generated by the local medical professionals and through the past and future
